KNOW YOUR HEALTH STATUS

Being aware of the health factors that can affect you personally enables you to do what's best for your health. That's why Live Well provides you with savings for completing the following simple steps that could change your life.

Rewards for participating in a Live Well Program are available to all eligible employees who are covered by a Company medical plan. If you think you might be unable to meet a standard for a reward under this wellness program, you might qualify for an opportunity to earn the same reward by different means. Contact WebMD at 1-800-947-HR4U (4748), option 3, and they will work with you (and, if you wish, with your physician) to find an alternative means for you to earn the same reward in light of your health status.

Participate in a Biometric Screening

There are three ways to take this simple step:

  1. Complete a physician form.
  2. Schedule an appointment with a lab.
  3. Attend an on-site screening (available to employees at certain locations). Register here.

If you have questions regarding a biometric screening, please contact: WebMD Customer Service at 1-800-947-HR4U (4748), option 3, Monday through Friday 8:30 a.m. to 8 p.m. Eastern Time.

A biometric screening is a set of tests conducted by a health professional to gauge your health status. Biometric numbers that fall outside of a normal or ideal range may indicate the presence of one or more health risks and should be interpreted by your physician. When you know these numbers, you can start making small changes that can have a big impact on your health.

Your biometric screening results should include a lipid panel (total cholesterol, HDL, LDL and triglycerides*), fasting blood glucose and blood pressure. This information can help you assess your risk for diabetes, hypertension, heart disease and stroke.

* Massachusetts state law prohibits LDL and triglyceride screenings.

Your biometric results will be uploaded in your Wellbeing Assessment and Personal Health Record on the WebMD Health Manager in approximately three weeks. Once your screening results are uploaded, you will receive an email from WebMD stating that updated information is available for your review.

The Wellbeing Assessment takes 15 minutes or less to complete, and your responses will provide you with a secure, personalized snapshot of your current health and the tools to create a plan to make improvements.
